Scheduling and Registration

Registration
When receiving care at Mosaic Life Care at St. Joseph - Medical Center, you will need to provide the following information to our registration team:

  • Your full name
  • Date of birth
  • Social Security #
  • Address
  • Telephone number with area code
  • Insurance coverage
  • Employer

This information, which is handled confidentially, ensures that accurate data is provided to those involved in your care.

Scheduling
Hours of Operation:  8:00 a.m. - 5:30 p.m., Monday - Friday
Your physician may have scheduled your test for you.  If you do not have an appointment date and time, you may contact Centralized Scheduling at (816) 271-1278 or 800-443-4515.

Pre-Registration
Hours of Operation:  8:00 a.m. - 5:00 p.m., Monday - Friday
Mosaic Life Care at St. Joseph - Medical Center can pre-register your information in our system in order to save you time during the day of your visit.

Call (816) 271-6579 or 800-447-6827, and a member of our registration team will confidentially take your personal and insurance information over the phone.  Please allow at least two days prior to your visit and have the date of your appointment and your insurance card ready when you call.

Pre-Admission Center
Hours of Operation:  7:00 a.m. - 5:30 p.m., Monday - Friday
If you are having a surgery procedure, you may need to complete Pre-Admission Testing (PAT), which can include tests such as lab work, an EKG or a chest X-ray. If you do not have a pre-admission testing appointment date and time, you may contact Centralized Scheduling at (816) 271-1278 or 800-443-4515.

When you Arrive
On the day of your visit, please be sure that you have:

  • At least one form of identification
  • Your insurance card(s)
  • Insurance referral forms (as required by your Insurance company)
  • Any written orders from your physician (order for permit to operate, and a History and Physical)
  • A list of current medications (including dosage and frequency)
  • A list of past surgeries
  • A list of medication allergies

If you have been pre-registered for your test/procedure, please report directly to the department where your test/procedure is scheduled.
 
If you have not been pre-registered, please stop at the registration desk located in that building, depending on where your test/procedure is scheduled.
