About Hospice

Mosaic Life Care - Hospice is a coordinated program of health care which works under the direction of the patient’s attending physician to provide medical, psycho-social and spiritual care for patients and families coping with life threatening illness. Hospice Care Services are provided by a variety of trained individuals who are part of the Hospice expert team.

Our hospice service provides life-affirming care to those coping with a terminal illness. Best of all, we strive to deliver the patient’s care within the comfort of their own home, so he or she can be surrounded by warmth, love and compassion.

Our goal is to provide the highest level of hospice care possible for persons who are terminally ill while offering supportive care for their loved ones. The sooner hospice care is started, the more effectively it can provide the best possible quality of life for you or your loved one’s remaining time.

The Hospice Standard of Care

The goal of Mosaic Life Care Hospice is to provide the best quality of care possible for persons who are terminally ill and offer supportive care for their loved ones. The great majority of Hospice patients are able to remain in their own homes with the care and support services offered by Hospice.

Pain and symptom control are the top priorities of Hospice, assuring that patients have a comfortable, peaceful and natural death. The sooner hospice care is started, the more effective the Hospice team will be in assuring quality of life in the remaining months and weeks.

Hospice patients are encouraged to retain control over the decisions affecting their lives. Hospice care at home is based on patient choice, and services are tailored to fit the wishes and needs of each patient and family.

Hospice Care at Home Services

The Mosaic Life Care Hospice team is dedicated and focused on you or your loved one through the following services:

Affirming life by improving the quality of care available for the terminally ill and their families.
Providing care in the home or in a nursing facility among familiar surroundings.
Providing care for a wide variety of diagnoses including heart disease, dementia, renal failure, chronic obstructive pulmonary disease (COPD), AIDS and cancer.
Providing care regardless of ability to pay, thanks to gifts from individuals, organizations, churches, synagogues, businesses, foundations and memorials.

Offering bereavement follow-up to caregivers of patients we serve.

Our Team

Your Hospice Care Team

A medical director and advanced practice providers are available 24 hours a day, 7 days a week to answer your questions and respond to your loved one’s needs. The hospice care team may consist of the following individuals depending on the patient’s needs, situation and doctor’s instructions.

Your hospice attending physician – the physician you choose to manage your hospice care.
Your hospice nurse – a nurse case manager will work under your attending physician’s direction to manage your physical symptoms. Other hospice nurses may visit you also. Our nurses share a weekend and night call rotation to assure that a nurse is available to you 24 hours a day, 7 days a week to respond to your emergent needs.
Your hospice social worker – a social worker will make an initial visit to learn from you what help you might need with things such as arranging for various community resources and providing emotional support to you or family members.
Your hospice chaplain – a hospice chaplain will also make an initial visit to get acquainted and learn from you what spiritual support you or family members might need. The chaplain can also call your own clergy or spiritual caregiver if you want.
Your hospice aide – a hospice aide will be assigned if you need help with personal care or light housekeeping. The aide works under the supervision of your hospice nurse and will follow the care plan you and your nurse agree upon.
Your hospice volunteer – hospice volunteers are available to help in a variety of ways. Your social worker will discuss possible volunteer needs with you. Volunteer assignments are made when you and your social worker agree that a volunteer would be helpful. As volunteers are giving their time, our staff needs advance notice to schedule them. The hospice medical director – our medical director serves as a consultant to your attending physician, advises the hospice team, reviews your medications and treatments, and is available to cover when your attending physician is not available.
The hospice pharmacist – our pharmacist reviews your medications and advises your doctor and hospice nurse on the best medications for your symptoms.
The hospice dietitian – our dietitian reviews the nutritional assessment your hospice nurse makes and is available to consult on questions related to your diet.
The hospice bereavement counselor – a bereavement counselor will be assigned to provide support to your caregiver following your death.
The hospice therapists – we provide physical therapy, occupational therapy and speech therapy consults if our medical director believes these would be helpful. Your hospice nurse will help figure out if you would benefit from any of these.
