Mosaic Life Care is gathering information as part of developing a plan to improve health and quality of life in the community it serves. We are reaching out to community residents to gather input so we may identify and respond to unmet health needs in the community.
The survey consists of 22 questions and should take approximately five minutes to complete. At the end of the survey you may provide your name and contact information to be entered into a random drawing for a $50 Amazon gift card. A total of 10 winners will be drawn.
The IRS has set new requirements for nonprofit (Section 501(c) (3)) hospitals that include completing a Community Health Needs Assessment (CHNA). Mosaic Life Care conducts this assessment not only due to the IRS requirement, but also because it is the right thing to do. The CHNA is vital to our mission of improving the health of individuals and communities in our region. This assessment will ensure that our efforts are directed toward the health issues that are of the most importance to the people we serve. Mosaic Life Care will conduct a CHNA every three years and adopt an implementation strategy to meet the community health needs identified through the assessment.
